    Hello Dr. Corenman –
About a year and a half ago I had a fall down the stairs. As a result I was told I have C5-C6 left paracentral disc protrusion with mild left canal stenosis. C6-C7 left foraminal disc protrusion with moderate left foraminal stenosis. Small syrinx at the C6-C7 level. I have been to physical therapy and have had a cervical block (which did wonders for me) The block lasted up until about 2 months ago – about a yeah and 3 months. Now, it the pain is back and is almost unbearable. The pain shoots down my arm almost feeling like someone is fileting a fish. More now than ever my left hand is going numb and I can barely tip my head back to rinse my hair in the shower. My question to you is, in your professional opinion do you think I will end up needing surgery? Is there another alternative?

Thank you in advance for your consideration in answering.

You can try another injection along with physical therapy but these injections can become less effective over time and frequency. You might need surgery but it is difficult to ascertain. If your physical examination noted significant abnormalities (motor strength weakness, reflex deficit) and your pain is impairing, you certainly are a candidate for surgery.
