    You appear to have a solid fusion of C5-7 and I will assume they are in proper alignment. There is no comment of the disc height or endplate integrity of C4-5. You could have neck pain from the disc or facets. If your shoulder/arm pain is right sided, this could originate from the C4-5 level (“There is severe right and no left neuroforaminal narrowing”).

    Without examining you, I could not ascertain where the pain is originating. You could consider facet blocks or even an epidural steroid to see if you gain relief.
