Hi—I am 3 weeks post L5/S1 microD. I had surgery urgently due to severe weakness of right foot. I had some calf cramping and tightening with walking about 10 days in, and a Medrol pack settled that down so that I could walk without the cramps. About two weeks in most of the calf pain was gone, and I increased my walking as I was feeling great. However, the last couple days I have kind of a constant tightness in the calf—almost feels like I did a really hard work out the day before. It is not unbearable but annoying and stretching is painful. Is this likely due to continued healing of the nerve and just has to settle? I saw my surgeon a few days ago before it started so did not mention. Of note is that while my weakness in the foot is better it’s still markedly weaker than the other side, so I don’t know if walking with the weak plantar flexion could make the rest of the calf sore. I also have soreness in front, on shin, which I didn’t have prior to surgery. It feels like a shin splint and ice helps.

Although calf “tightness” after a microdiscectomy is not unusual, one of the other possibilities is a blood clot in the calf (DVT). I would call your surgeon and ask if that could be a possibility.
