During a discectomy, the surgeon not only has to remove the nucleus (the jelly) that is in the canal pushing on the nerve root but has to go into the disc to remove any loose jelly that could come out after surgery and cause further nerve compression. There will be a through and through tear of the disc wall (annulus) that allowed the jelly to exit in the first place. This hole is the pathway that allows the surgeon to reach through the disc wall into the inside of the disc.
