    Dr Corenman, about 9 weeks ago I had a anterior c5 c6 fusion with bilateral c6 foraminotomy. The tingling in my arm has subsided and I some strength back but my main reason for going in for surgery was for pain relief. My main problem is that it feels like a facet joint on my left side is pretty much bone on bone so the pain gets worse as the day goes on and is almost unbearable. If the offending facet joint was on the level of my fusion will it be immobile now or will there still be some movement? If so could this still be why I’m getting so much pain when moving my head? Any advice would be greatly received as I’ve been dealing with this along time now. Regards Matt

    Pain can be generated by disc, nerve and facet. If the operated level was a neck pain generator, whether from disc or facet, the fusion should cure the pain as the levels stops moving and it is movement that induces pain. However, if another level is also responsible, you will only obtain partial relief.

    Thankyou for your reply. Obviously the 2 vertebrae haven’t fused together yet as im only 9 weeks but are you saying the joint is now fixed and that there is no facet joint movement? I thought as the joint becomes more soild this might limit motion further? This would then suggest as you say the offending joint is on a different level and that my surgery has fixed the arm weakness but not my main problem which is constant pain in my neck from movement.

    In your expertise would a CT scan show the bone structure of the facet joint better than an mri? I’ve never had a ct and a lot of what I’ve read shows that ct can really help determine facet problems. My surgeon is so hard to get hold of I feel I’ve been left out in the cold really and I’m in so much pain. Thanks again

    An MRI will demonstrate significant facet disease if the bone has been damaged (using the STIR images). If the facet is reasonably intact without severe bony damage, a CT scan might not demonstrate anything unusual. 9 weeks should be enough time to determine after fusion, pain relief so I would suspect another level could be involved.
