The X-ray reports note typical degenerative disc disease of both your cervical as well as lumbar spines. The basic start of care is with physical therapy or chiropractic care. Find a well respected therapist or chiropractor who involves you with self-directed exercises so that once you are discharged from care, you can continue to treat yourself.

    Dear Dr,

Thanks a lot for your response.I have consult a Dr yesterday, he told me that I have to correct my posture and he suggest some exercise.
Do you think this is the begining stage and can it be rectify by exercise or I have to go any surgery.

Exercise is the cornerstone for rehabilitation. Many times, simply stretching and strengthening the spine will relieve the symptoms. Surgery is for incapacitating and non-treatable symptoms (see website under “When to have lower back surgery” or When to have neck surgery” to understand this concept).
