    Dr Corenman. I had a left sided tlif l5s1 done back in December 2023. Hardware was placed only on the left side I had a couple set backs from very minor events during the first 6 weeks. Always in the same area left side and felt very deep pain. Then through June I felt good. Around the 1st of July I was sitting back into an incline chair and felt a crunch on the left side down near the surgery. It did not feel good. About 3 days later I felt completely destabilized and had and continue to have very bad sciatica and back pain. Nothing works. Interestingly my screw placement into both the sacrum and l5 veterbre the screw threads appear to be almost flush what the end plates. Even sticking through a little. I have not seen anything close to to this on anyone else’s scans. I think my fusion failed because of this. I know it has. My surgeon says it all looks good. Good fusion. A second opinion said the screw placement does not look good and maybe on the l5 nerve. I may need a consult. Any initial thoughts? The screw placement looks way too close to the bone surfaces not to fail

    Dr Corenman. I looked at my mri to see where the peek cage was placed. It was not anterior or centralized. It was all on the left side (did not even make midline). As if the step after insertion to set it properly was never done.
