    Good evening Dr.
    I’m writing to ask , does this appear to be bertolotti syndrome? I was just told “nerve pain”.
    Ct scan states ;
    There is a transitional lumbar sacral anatomy with left sided partial sacralization of l5 with fusion of the left l5 transverse process with the sacral ala.This contributes to altered bio mechanical stress and advanced right sided sacroiliac joint osteoarthritic changes. This also contributes to a leftward curvature of the thoracolumbar spine.
    Hypertrophic changes with potential contact the transiting right s1 nerve root at l5-s1.
    Left sided neuroforaminal narrowing at l4-5 potentially impenging on the exiting left l4 nerve root.

    Classic Bertolotti’s syndrome is a movable painful articulation of the transverse-alar articulation of L5-S1 on one side. You have a fusion of that articulation {“left sided partial sacralization of l5 with fusion of the left l5 transverse process with the sacral ala”). This leads to overload of your sacroiliac joint and “arthritis” on the opposite side (“advanced right sided sacroiliac joint osteoarthritic changes”).

    You also have degenerative changes above. All could lead to various symptoms.
