    Dear Dr Corenman,
    I’ve been having burning middle back pain for about a year-it mainly hurts when I lean against something – the back of the chair, pillow etc. I had MRI a year ago, which showed some intake on C7, to investigate this I had a nuclear bone scan and spect ct and it was decided that I have a synovial cyst on C7. I saw neurologist – he didn’t think it was neurological issue, physio thought it could be from neck but his treatments made it worse, painkillers don’t help and its progressively worse in the afternoon when I sit on the office chair – any idea what can cause this type of contact/ pressure pain??

    Leaning back is called extension. This maneuver loads the facets and narrows the spinal canal. Burning pain can be generated by loading degenerative facets so if the pain is significant enough, I would start there with a good examination and possibly diagnostic blocks.

    Thank you very much for your reply, this burning pain happens when my back touches the back of the chair/sofa for prolonged periods( more than 20 min) , not from leaning back, it’s a contact pain.
    I can be lying down touching the back of the sofa or sitting down touching the back of the chair,.
    Any idea what that can be?

    Local pain with skin pressure have a myriad of causes which include spine origin. If your only pain is contact pain, find a neoprene belt to wear when you plan to have prolonged skin contact. The closed cell foam can reduce contact pressure. You can use lidocaine cream or a lidoderm patch over the tender spot too.
