    MRI CERVICAL W WO CONTRAST –

    1.Surgical fusion C5-C6 and C6-C7 disc spaces. The disc line at C6-C7 remains visible although
    there is no evidence of chronic abnormal motion.

    2.Tiny broad-based disc protrusions at C3-C4 and C4-C5 disc spaces.

    3.Mild multilevel facet spondylosis.

    4.Stable examination.

    Narrative
    MRI CERVICAL W WO CONTRAST

    HISTORY: Neck pain and bilateral upper extremity pain, numbness, and weakness.

    COMPARISON: Cervical spine MRI performed July 3, 2018.

    TECHNIQUE: Multiplanar multisequence magnetic resonance images of the cervical spine were
    obtained at 1.5T in the absence and presence of 18 mL intravenous gadolinium contrast.

    FINDINGS: Central canal diameter is congenitally generous. No cord signal or enhancing abnormality
    was found.

    At disc level C2-C3, no disc protrusion, canal, or foraminal stenosis is present.

    At disc level C3-C4, disc height is preserved. A small disc protrusion flattens the thecal sac
    slightly narrowing canal diameter. The neural foramina are normal.

    At disc level C4-C5, disc height is preserved. A tiny disc protrusion scallops the thecal sac
    minimally narrowing canal diameter. Facet hypertrophy minimally narrows the neural foramina.

    At disc level C5-C6, ferromagnetic artifact partially obscures the vertebrae after ACDF with
    intervertebral graft. Complete bone fusion spans the disc space. Posterior bridging ossification
    scallops the thecal sac slightly narrowing canal diameter. Uncinate spurring minimally narrows the
    neural foramina.

    At disc level C6-C7, ferromagnetic artifact partially obscures the vertebrae after an ACDF with
    intervertebral graft. The disc line is present. Posterior marginal osteophytes scallops the thecal
    sac minimally narrowing canal diameter. The neural foramina are normal.

    At disc level C7-T1, disc height is preserved with no disc protrusion or canal stenosis. Facet
    hypertrophy moderately narrows the neural foramina.

    My neck pain began with a car accident in 1990. In the accident my car went up in the air rolling 4 times before coming down on the roof of the car which then slid 280ft before stopping. My neck was crunched when the car came down on its roof. My lawyer never sent me to a doctor just one year in chiropractic therapy. Which left me every time with a migraine. After that I was having many headaches and when I turned my head for any amount of time this burning pain would begin and get more intense like someone tightening a rubber band in my neck. So I learned to turn my whole body if I needed to turn and look at something. I went for years dealing with migraines and pain with no insurance to have anyone tell me what was happening. Around 2002 I had insurance to do something and went to see spine doctor. He told me that I had several bulging discs. I did physical therapy for a year then a year of injections in the neck. When finally nothing was fixing the pain or the trouble moving my neck so this doctor went in with surgery to fix it by putting plate in my neck. I soon realized that something was wrong because the pain worsened, it was now running down my neck into my left shoulder and arm and wrapping underneath into my armpit. So I went to medical doctor to see what to do and was put into pain management. Since then I have fallen which left me having to have a reverse total left shoulder replacement. So now I have pain that medication sometimes controls to a point and sometimes not. I have like 80% muscle weakness. My doctor told me that the neck surgery left me with permanent nerve damage as well as the muscle running down the right side of my neck and continues down my back has disintegrated leaving my left neck muscle to due all the work. I now live with alot of pain in my neck shoulders and back along with so much muscle weakness. As well my left shoulder arm and leg hurts all the time. I feel as if I can’t rely on anything on my left side for the numbness and tingling and weakness. And headaches and pain in my neck all the time. I feel exhausted 24/7 and never get a full nights sleep. My family doctor tells me that my muscles in my body spazm all the time. He has me taking muscle relaxers and I can tell that they are working but not enough. I’m 56yrs old and feel 90 and just tired of living this way. I have no life because it hurts to walk stand and sit. Basically I live my life in bed and that’s not a life I want because I have grandchildren I can’t do anything with.     You then note “When finally nothing was fixing the pain or the trouble moving my neck so this doctor went in with surgery to fix it by putting plate in my neck”. I assume this was a fusion (ACDF). Of how many levels? Have these levels been verified to be fused through X-rays, a CT scan or an MRI?

    “the pain worsened, it was now running down my neck into my left shoulder and arm and wrapping underneath into my armpit”. Again, any imaging studies performed after surgery that could have given some information regarding the decompression or new levels?

    “Since then I have fallen which left me having to have a reverse total left shoulder replacement. So now I have pain that medication sometimes controls to a point and sometimes not”. Sometimes, shoulders can continue to be painful after a joint replacement.
