Tennis elbow is not nerve compression related. Headache typically (if generated by the cervical spine) is generated by the upper and not lower facets (with some exceptions). Left chest pain could be from the C4 nerve (not noted on your MRI findings). Now the pain into the shoulder and down the fingers could be related to C5-6 or C6-7 (not C4-5).
Pain management might be a good next step.
