    Hello Dr. Corenman

I am hoping you might be able to help me understand my results cause my PT Dr and Regular Dr have me more confused. Below is the report I got from MRI

    MRI C-SPINE W/O CONTRAST

At C2-3 there is normal disc space height and signal. No spinal Stenosis

At C3-4 there is normal disc space height with subtle loss of disc space signal. No spinal stenosis

At C4-5 there is mild loss of disc space height and signal. Small anterior marginal osteophytes. Minor posterior disc bulge. No significant spinal stenosis

At C5-6 there is mild loss of disc space height and signal. Small to moderate anterior marginal osteophytes. Posterior osteodiscogenic material effaces the ventral epidural space causing moderate central spinal canal stenosis. Uncinate degenerative changes contribute to bilateral C6 foraminal stenosis.

At C6-7 disc space height and signal are normal. No spinal stenosis

    Impression

DEgenerative disc disease and spondylosis with spinal stenosis as described. This is most prominet at the C5-6 level where there is likely small central disc extrusion and uncinate degenerative changes contributing to foraminal and central spinal canal stenosis.

What does all this mean? I get 2 different stories.

PT said she couldnt help me anymore. It was made worse with traction. She said think about surgery.

I was denied at the referring pain clinic.

Regular dr said she didnt know what to do with me cause she doesnt see anything wrong.

My pain is in my neck shoulder shoulder blade arm hand (left side). When I try to bend my head back it hurts plus pulls in my upper arm and goes tingly and shooting pain. Since traction it has gone from just numbing my middle to pinky finger to thumb now goes dead it feels like I sat on it and it went to sleep. I am also getting light headed when I move or at times sitting and move wrong. It feels better when I lay with cervical pillow under neck and pillow under my arm. When I sit up from laying down it starts to hurt and feels like my arm is pulling off. I am not pain free even with meds. What is happening what is that MRI saying? Should I get another dr? Please help if you can I am about to lose my mine here.

Thank you for your time in advance and any info you may give me.

Your symptoms are consistant with foraminal stenosis (see website to understand this disorder). “My pain is in my neck shoulder shoulder blade arm hand (left side). When I try to bend my head back it hurts plus pulls in my upper arm and goes tingly and shooting pain”.

When you extend (bending your head back), you narrow the formamen (the nerve exit hole) which pinches the nerve. Typical symptoms of a pinched nerve are tingling, pins and needles and pain down the arm in the distribution of the nerve.

Your MRI findings (“Uncinate degenerative changes contribute to bilateral C6 foraminal stenosis”) mean that you have foraminal stenosis
that compresses the C6 nerve root. See the section “symptoms of cervical nerve injuries” to understand how compression of that nerve manifests.

You need to find a spine expert as your current doctors do not understand this disorder.

I went to my reg. dr and asked for a referral to neurosurgeon she said not until I go to pain clinic which I finally got appt for new patient intake. I told her I need to know what MRI means her reply was ” you have a bulging disc and narrowing of the spinal column. I dont understand why I have to wait til I see Pain clinic? Is this something that is no big deal?

The question of “big deal” or “no big deal” has to do with your symptoms and motor weakness presence. This might be a good move as long as the doc you see understands these disorders.
