Hello Dr. Corenman,
I have had numbness in my hands and feet, along with tingling for almost 8 months now. I have also been experiencing some loss of balance when I first get up from a sitting position. Pain in my shoulders, radiating down my arms. Constant back pain. I am a relatively healthy 62 year old female, who hikes, hunts, and is still works full time. Over a year ago I had a meningioma removed from L8-9. Recently I had an MRI done on my neck, without contrast.
The results of the MRI are as follows:
Craniocervical Junction/C1-2: Unremarkable
C2-3 No significant canal or foraminal stenosis
C3-4: Slight anterolisthesis. Shallow riding to the right. This abuts the ventral cord. Right foraminal spurring with mild to moderate right foraminal stenosis.
C4-5: Disc osteophyte complex abutting the ventral cord. Ventral cord contour deformity noted. Loss of ventral and dorsal cerebral spinal fluid spaces. Moderate left and mild right foraminal stenosis.
C5-6: Disc osteophyte complex abutting the ventral cord. This encroaches on the neural foramina bilaterally. Mild left and moderate right foraminal stenosis right.
C6-7: No significant central canal stenosis, Mild left foraminal narrowing.
C7-11: No significant canal or foraminal stenosis.
IMPRESSION:
1. At C4-5, disc osteophyte comple with cord contour defrmity. Loss of the ventral and dorsal CSF spaces. Moderate left foraminal stenosis.
2. At C5-6 shjallow disc osteophyte complex abutting ventral cord. Moderate right foraminal stenosis.
3. At C3-4, right foramminal spurring with mild to moderate right foraminal stenosis. Shallow ridging abutting the ventral cord.
4. Several indeterminant left thyroid cysts versus nodules. This can be correlated with thyroid ultrasound.WHEW!!!
Now with all this, would you recommend surgery???

Loss of balance along with paresthesias (pins and needles) and numbness in all four extremities makes me think of myelopathy immediately. Pain in shoulders could be related to the degenerative process and foraminal stenosis (C4-5 “Moderate left and mild right foraminal stenosis”).
It is the central spinal stenosis that worries me (“C4-5: Disc osteophyte complex abutting the ventral cord. Ventral cord contour deformity noted. Loss of ventral and dorsal cerebral spinal fluid spaces”).
Your first need a good physical examination to determine if you have long tract signs (findings consistant with myelopathy) along with a thorough history and a careful evaluation of your images. You might need surgery but I cannot say from this distance.
