    For multilevel radicular symptoms in a patient with cervical spondylosis, which treatment is best for the patient? Is it ACDF or Posterior cervical foraminotomy?

    This question has many qualifications. Is the nerve compression from a disc herniation or a bone spur? If a disc herniation, does the herniation press under the spinal cord (it is anterior to the cord) or is it in the foramen only. If it is in the foramen only, either surgery could be contemplated. If it is under the cord, only an ACDF should be contemplated in my opinion.

    Is there bone spur associated? If so, is this spur from uncovertebral joint hypertrophy (see cervical radiculopathy) or is it from a spur off the facet? If it is off the facet only, a posterior foraminotomy can be successful. If only uncovertebral joint hypertrophy, generally an ACDF is recommended. If the disc still has reasonable height, an artifical disc can be considered (see bryan disc).

    I apreciate your response.There are compressions from uncinate spurs and discal material anteriorly. I work in resource poor environment. I retract the neck with a bandage as i do not have casper pins and power burrs. I therefore use pituitary rongeur to remove the uncinate spur and the disc material through anterior discectomt.I shape a cortico-cancellous graft and put it in the intervertebral disc space. The patient is left in a cervica collar for six weeks. In the future, i hope to acquire some tools so that the procedures will be more elegant.
