You have an isthmic spondylolisthesis of L5-S1. See the website for description. You have developed foraminal stenosis (again-see website)at this level which is quite common. This is what is causing your back and leg pain.

Conservative measures such as physical therapy (core strengthening and flat back posturing) can sometimes be helpful. Epidural injections can also give relief, sometimes long term.

If these measures are ineffective, you would need surgery. Generally a TLIF (see website) is the most effective procedure for this disorder. Review the TLIF surgery on this website.

Well-having never taken your complete history or examined you, I can only speculate. Normally, 50% of patients get complete relief of back and leg pain. Most patients have 75% reduction of their back pain and near complete elimination of their leg pain. 90% are satisfied with their results,

There should be no “side effects” from surgery performed properly.
